A 38-year-old man with a 5 year history of ulcerative colitis presents for follow-up. He was diagnosed at age 33 when he presented with diarrhea, bleeding, urgency cramps, and weight loss. He was treated with a short course of prednisone and has never been hospitalized for his disease. He was initiated on 4.8 grams of mesalamine as he stated he “would never take steroids again.” He did well for 18 months and his mesalamine dosing was decreased to 2 grams per day for maintenance.
