Capsule endoscopy is now becoming the standard investigation for patients with occult gastrointestinal bleeding when upper and lower endoscopies are negative. While the spectrum of lesions found on capsule endoscopy and their implication have been studied, the long-term outcome of patients with a negative study are not well described. This study reports the outcome of a cohort of 42 patients in whom long-term follow-up greater than 1 year was obtained. Taken as a whole, significant lesions were identified in 57% of patients. The overall rebleeding rate was 28%, and patients with a negative study experienced only a 11% recurrence as compared with 42% in those whom a lesion was identified.
