Where the Twains Should Meet
The article by Yachimski et al1 aimed to identify predictors of treatment for patients with Barrett's esophagus (BE) and high-grade dysplasia (HGD) or intramucosal carcinoma (IMC). The interesting finding is that treatment choice was influenced heavily by the initial physician evaluating the patient: gastroenterologist versus surgeon. Unfortunately, the reasons for this dichotomy cannot be ascertained from the study. It is perhaps noteworthy that none of the authors were surgeons. I believe the real value of this article should be a plea for collaboration. Should not Barrett's esophagus be where the twains meet?
